Torpedo VVV / MEGA Tank

I am running the vvv, with the 510 adaptor, mega atty & tank attached, and am having a couple of issues with it...

(1) When the atty is tight - the device 'whistles' when used, and if you loosen the atty a touch, it leaks

(2) Also, I have recently started to get fluid return in my mouth. Tried a new cap on the tank, no improvement. Tried a new tank... still no improvement. Tried a new atty, and still the same. Not sure what else I can do.

Could this possibly be anything to do with the adapter?
I dont have a spare to try, and have only have this second issue since the 1st adaptor broke, and it was replaced.

Anyone else using this setup and had similar problems? Any advice welcomed.
